The Program Navigator for the Digestive Health Services (DHS) Colon Cancer Prevention Program (CCPP) works under general supervision of the Medical Practice Manager and is responsible for the coordination of patient referrals to the program. Responsibilities include working incoming referrals, patient scheduling, obtaining patient information, providing the patient with information and instructions related to the procedure, verifying eligibility, and full registration of patients in the electronic medical record (EHR).
